Job Summary:
We are seeking a detail-oriented and compassionate Remote Certified Nursing Assistant (CNA) or Medical Assistant (MA) to join our team, supporting patients and healthcare providers through remote patient monitoring and chronic care management. The ideal candidate will excel at reviewing patient data, documenting information accurately, and providing timely communication to care teams. This role is fully remote, working on the backend to help ensure patients receive the highest standard of care.

Responsibilities:

  • Review and monitor patient health data through remote platforms.

  • Document patient information accurately and promptly in electronic health record (EHR) systems.

  • Communicate important updates or concerns to nursing staff and healthcare providers.

  • Assist in patient outreach for wellness check-ins, reminders, and care coordination.

  • Participate in quality improvement initiatives to enhance patient outcomes.

  • Maintain confidentiality of patient information and uphold HIPAA regulations.

Requirements:

  • Certified Nursing Assistant (CNA) or Registered Medical Assistant (RMA) certification required.

  • 1–2 years of healthcare experience, preferably in remote patient monitoring, chronic care management, or a similar role.

  • Strong communication and interpersonal skills.

  • Ability to work independently and manage multiple priorities in a fast-paced environment.

  • Proficiency in electronic medical records and comfortable using healthcare technology.

  • Bilingual in Spanish preferred.
