Spectrum Healthcare Resources is seeking a dedicated Registered Nurse (RN) - Disease Management to join our team remotely. This role focuses on providing high-quality telephonic care, health coaching, and education to patients with chronic conditions, helping them improve their health outcomes and quality of life.

Conduct telephonic health assessments and develop personalized care plans.
