Registered Nurse - Case Manager- REMOTE options ID-6927

Unitek Learning is seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Case Manager. This remote position offers flexibility while allowing you to make a meaningful impact in patient care. As a Case Manager, you will be responsible for coordinating patient care, evaluating treatment plans, and advocating for patients to ensure the best possible outcomes.

Responsibilities:

Coordinate and manage patient care plans, ensuring that all necessary treatments and services are provided.
Conduct comprehensive assessments to evaluate the patient’s needs, including physical, emotional, and social aspects.
Work closely with healthcare providers to develop and adjust care plans based on patient progress.
Act as the primary point of contact for patients, families, and healthcare providers to ensure seamless communication.
Provide education and support to patients and their families about conditions, treatments, and recovery plans.
Ensure that patients receive the appropriate care in a timely manner, minimizing hospital readmissions.
Maintain detailed and accurate case documentation to ensure compliance with regulations and policies.
Collaborate with interdisciplinary teams to assess patient progress and adjust care plans when necessary.

Requirements:

Current and active RN license in good standing.
Minimum of 2 years of clinical nursing experience in a healthcare setting (hospital, clinic, or similar).
Strong knowledge of case management principles and healthcare services.
Excellent communication skills, both written and verbal.
Ability to work independently, manage time effectively, and meet deadlines in a remote environment.
Proficient in using case management software and electronic health records (EHR).
Certifications such as CCM (Certified Case Manager) are a plus but not required.
Ability to pass background checks and drug screening.
